I'm trying to figure this one out, too.  I called the guy at the link above, but he was no help.
I can't find anything that covers more than ER visits outside of the issuing state.
OTOH, I have been pretty lucky so far with good, cheap out of pocket care on the road.
AFAICT, you just have to pick one town in one state, try to do the majority of your healthcare stuff when you are in that town, and pay out of pocket everywhere else.
The difference under Obamacare is that if you have a preexisting condition you CAN GET insurance for that one state and for ERs, and for a cheap price. (Pre- Obamacare, Blue Cross quoted me over $1000/month. Now it's about $300.)
