Sounds to me like the macerator pump impeller has failed. That happened to me on my Roadtrek. The impeller is made out of some sort of rubberized plastic so an obstruction won't burn up the motor, but it doesn't stand up to well over time. There will be a different sound it makes if the impeller isn't working - when it's doing it's job you will hear a pretty distinct noise when you start it, otherwise it may be so quiet that you don't even know if it's running. The motor itself is very quiet and all the noise you hear is generated by the spinning impeller if it's doing its job.
The good news is the macerator pump can be removed, repaired and replaced without having to dump the tanks provided the dump valves remain closed and don't leak. I did mine myself, and it's not all that difficult but there is very little room to work. I had to drive up on some planks to make more room to work and it's a bit unnerving to be working on the sewer system knowing that there are two tanks full of waste just inches above your head!
