Norcold Class Action Lawsuit Forms

I tried the web site and was able to bring up 70+ pages of everything
but the claim form. I then tried the phone number and none of the choices directing me to the claims forms.
Does anyone know where I can obtain a form.

This all comes on the heels of us returning home from a trip and having our refrigerator die on our last day out. All of this after having the recall "taken care of" on three separate occasions due to faulty replacement modules. This will be the fourth attempt at having them fix a problem that they should have corrected years ago.

Hey Paul, Is this another class action against this same item? The original, and I thought only, action was completed in 2014. See below, complete with link.
