Just out of curiosity---why would the one brake pad be so much more worn than the others? Is that something to be concerned about?
The wear was even on all pads, its just that the lining material fell off the LR, outer pad.
I suspect a flaw in the bounding process.

I'd say that you got lucky in that the bond failure didn't result in the brake pad material falling off or being spun off the metal back plate when the brakes were applied. Replacing metal to metal scored rear brake disks can get expensive.
