Follow-up.....
So I finally got time to look for the problem and found a very simple fix.... the bolts that connect the slide motor to the connecting shafts had sheared. Half the bolts were still in the holes, but the other halves were laying in the bin underneath. I replaced them with the proper Grade 8 bolts and locknuts, tightened all the others and now everything works just fine!
