Rosie, go ahead and build the insulated place for him, so he can discover it and become accustomed to it. Gradually move his food from where you are currently feeding him to the new digs, so he'll connect you with both food and shelter.
Each time you see him, talk gently to him. He may never respond, but he'll learn to tolerate your presence and be less skittish.
Don't be in a hurry to capture him and take him to the vet, since you don't know if he's completely feral or not. Let him get accustomed to a stable routine: cats love their routines. Food, human talking gently, shelter, human talking gently, food, etc. Only when he's decided this will be his routine should you change it with a capture.
And whatever you do, don't capture him inside the shelter -- he'll likely never return to it.
