The car doesn't have to be turned on, just the key has to be clicked to keep the cars radio on position, which is ok and wont drain the battery.
I'm probably late to answer... I have a Pepwave Surf on the Go router that I use to connect to a ZTE Mobley over wifi. This allows the Pepwave to distribute the signal as long as the two are close enough for the router to get a signal from the car to actually distribute.
