Has this ever worked properly? In addition to the SWM power inserter, do you have the SWM8 switch as well? Assuming you do, that could have gone bad or one of the connections may be loose/corroded. It could also be the LNB itself going bad. That's a fairly old antenna so all connections should be checked, all outside coax connections should be shiny clean. Hopefully some dielectric grease is in all F connectors.
