We have the same truck and hitch. Sorry for your troubles. We had the dealer put in a spray on bed liner and then had the hitch put in elsewhere. I never thought to look to see if the bed was actually level but I haven't noticed any problems.
Is your hitch torqued down properly into the pocket? Was your turnover ball hitch installed properly? Perhaps a spray on bed liner would add just enough strength to avoid additional damage.
Your pads need to be flipped as previously mentioned.
