I have to check again, but in my area the exchanges are substantially more than the cost of a refill. I think the last time I compared a refill for a 20lb "grill" tank was $11.00, and an exchange was $18.00. Although, I wouldn't be surprised if I have the numbers mixed up.
In talking to some hardware/garden center owners who do (or used to do) refills, the exchange system is taking over the refilling. Due mostly to regulations and insurance costs. I wouldn't be surprised if in the future the only places you can get refilled will be at campgrounds and at a propane supplier's building.
