There was a poor connection at the plug, which could be caused by any number of things: dirt/scorching of the plug prongs, similar on the mating outlet connectors, a loose outlet, or poor connections to the wires in either the outlet or the heater plug. Perhaps the most common are the loose outlet and the damaged/dirty plug prongs.
I make it a habit of checking the plug on my electric heater after turning it on for a minute or two in order to be sure it's not heating up. A couple of the outlets in my RV are on the looser side.
