This post reminded me of something I was told of years ago working in Daytona Beach in my trade (elevator mechanic). One of my co-workers answered a trouble call where a dog was electrocuted in a condominium building as it stepped onto the elevator when it's owner went to take it for a walk. The cause was a broken ground wire that went un-noticed until one of the fluorescent ballasts burned out. Without the ground wire to ensure the breaker would trip from this failure, the metal frame of the elevator was then energized by the 110 volt light feed. Luckily it was the dog that paid the price rather than someone coming in off the beach with wet feet....
