Nope. There are pictures of him standing by the side of the truck doing paperwork after the crash, if some medical issue were involved he would have been sent to the hospital in an ambulance to get checked out. Unless by "medical emergency," you mean he was trying to pee into a Gatoraide jar.

Well, simplest explanation, preponderance of evidence, stuff like that. This isn't a courtroom, assuming the cause was the driver of the vehicle which rear ended someone is a perfectly reasonable conclusion unless/until shown otherwise. It is the cause in the vast majority of rear-end collisions. Other explanations require unlikely coincidences.
