Come on folks.
ALL of you who are suggesting that he "roll his own" have been around enough on this forum to KNOW that a lot of people are not capable of doing electrical stuff themselves.....safely.
Especially someone who has to ask the question in the first place.
So....for most people in the above category the best thing to do is buy a ready made adapter. It just IS.
