If you have a way to test (or substitute) the FAN RUN capacitor please do. My guess is that it's passably OK since the fan starts, but I do know that if it's of the wrong MFD, uF, MicroFarad capacity, one outcome can be an overheating motor.
Chances are, motor shot. Some replacements come with a new capacitor, others don't. With your motor's history, I'd suggest a new capacitor even if you need to buy it separately.
