I'm saying it's starting in 2nd gear or a timing issue over a plugged cat. How does it accelerate when going down the road? If you're driving 45 and you step hard on the accelerator does it accelerate like normal, or is it equally lethargic? If lethargic I can imagine the cat, if normal I would look at the transmission.
