Perhaps an abstract approach as is noted in a response above would be most revealing.
By Certified US mail return receipt requested
Ask the vendor if the failed unit can be shipped to them for analysis and response similarly by mail letter.
State the reason, as being WHAT CAN I DO TO AVOID FUTURE FAILURES OF THIS KIND? Where the failure occurred?. By what manner?
Such a request puts them in a bind. Failure to respond altogether is a no-no when presented to UL and ETL (I imagine Canada's CSA has a similar policy).
Omit threats directed at them for now. Your experience doing so will not only help you but a lot of other folks :)
