I went to the local shop where my truck was towed Friday afternoon and had a long conversation with shop owner, much of it based on many of your responses here.
As a first step we decided to eliminate the microbe and tank trash problem by upgrading the tank with a new larger 60 gallon aftermarket unit. I get added advantage of 20 more gallons than OEM tank too.

The tank should be here by Friday for him to do the install probably next week. When it gets here and I get the information on the condition of the tank, I'll report back.
Hopefully this is all that is wrong, but a full fuel system check will be conducted.
