I know of a hot shotter who got +400k miles on his 6.2 flat bed truck.
He started to get CEL's and the engine was running sluggish so he took it in. The tech removed the valve covers and noticed galling on the cam lobes. At that point it was determined it was better to just replace the engine (418k miles) with a remanufactured long block. The truck still had the original transmission.
