My truck doesn't have any vibrations while driving empty or towing, but I've read some report it during take off while pulling a very heavy load.
These trucks do come with an engine brake, but are not the best performers, but the tranny also helps in this regard. The engine brake and tranny work in unison when the Tow/Haul mode is selected. I live in a relatively flat part of Texas so engine braking is not really used so I can't share first hand experience.
Not really sure what to say about a duel exhaust. Perhaps you confusing the standard duel tail pipe on the passenger side?
