In regards to setting the parking brake and people not always using it. Obviously using both is the best option of course but just for the purpose of this discussion, I don't think the parking break is really as effective as leaving the vehicle in gear, at least in my own experience. In my VW Jetta TDI (Diesel) 5 speed with 310k miles on her, the parking brake is very tired. I still set it of course out of habit but lately I've also had to make double sure that I've left the car in gear when on a slope. Otherwise, even with the parking brake engaged there can be movement. I've never had her roll on me but getting in and out of the car causes enough movement that it reminds me to check to make certain I leave it in gear.
