Hitch jack fully extended: Should I be nervous?

Personally I would use more wood and skip the shoe. Without the shoe the jack can sink a little into high side of the wood rather than forcing everything into a slight bend. If you can build block cribbing that ends up closer to level then the shoe would be fine but as it is, biting into the wood just traps the block more firmly, in my opinion.
