There are essentially 4 different "5.4" V8's that have been around over the years.. 97-98 was the original 5.4's. I had a 97 and it did 255,000 miles when I traded it and still had a lot of life in it.
99-03 in the F150s was with what they called a "PI" head. It had a "Performance Improved" head design, but was still a 2 valve setup.
During that same time (99-03?), the Lincoln Navigators and Lincoln Mark LT "F150" came with a 4 valve version of the 5.4 and I don't recall hearing many issues with that one?
04-09... Yes, those are the 3 valves and at least with the early models, that's all you heard about was the "Cam Phaser" issues with them.. I think the last few years were better, but I would not touch one of those era 5.4's..
Anyway, not all 5.4's are the same... ;) Sure, some spit out spark plugs and some didn't... Some cam phasers went out and some didn't...
I'm still in a F150, and it's the infamous "Ecoboost" and some are reporting problems with them and some aren't... So far, I'm doing just fine... But, I did go ahead and get the Ford ESP plan for up to 100,000 miles on it... ;)
