I read the order. It is horrifying and thank goodness there was a government agency to shut the company down. The company changed the manufacturing and the spec from what had been approved without getting reapproval, did not perform the testing at the required pressure levels and when the issue surfaced and the DOT came calling and inspected and tested a bunch of cylinders that the company said had already been tested, more than 30% failed. All in all, per the order, this company was a really bad actor.
