For a C-clip axle to be worn enough for the clip to fall out you'd notice other issues as well such as leaking axle seals, a lot of clunking from the diff and very low brake pedal on disc brake vehicles. It would have to wear an absolute ridiculous amount for the clip to fall out.
And just because a roller bearing is pressed onto an axle that doesn't necessarily make it strong. It has a retainer ring pressed on behind the bearing to hold it in place. Old stock car racers used to tack weld that to the axle to keep it from falling off and losing the axle.
