I got quotes from several different manufacturers and all of them were willing to build one with an extended tongue. Just make sure to also order one with a jack that pivots on its mount. Once you've got the trailer hooked to the truck, pivot the jack out of the way - it will rest alongside the tongue instead of sticking up above it.
Good point brought up in the one post above - if the back of your camper hangs down really low then an extended tongue will not work because the pivot point will be at the ball hitch. In that case the Supertruss is your only option as it moves the pivot point to the end of the camper. In my case that's not a problem because, though the Bigfoot extends out past the bed of the truck, it does not hang down at all.
