Within minutes of my sending contact information as was requested, I received a call from the care team. This was on a weekend no less. After a few calls over the last few days this issue has been resolved. I would like to thank those who worked to bring this to a conclusion. I'd like to add that each of the people I spoke with were extremely nice and followed up as promised.
