Yup, tensioners (Cat, Cummins, DD, etc) can fail in one of THREE ways:
Bearings
Tensioner spring
Dampner
BTW, I like Gates tensioners.
Particularly if you have a rear radiator (i.e. labor to install), replace the tensioner when replacing the belt. Only one more bolt to R&R-- the rest of the labor is "included".
