My guess is that if the propane tank was completely empty, they unscrewed the vent valve to far, and perhaps it came off?
I have had this happen in places where they do not fill a lot of propane tanks, when I bring in a portable tank that is completely empty. They unscrew the vent until some propane comes out, but on a empty tank, they do not stop at one turn.!
So after they open the liquid valve and start adding propane, then it shoots out a tremendous amount of propane gas (because the vent was wide open and should not have been open more than a tiny amount). The auto shut off valve will stop filling the tank before liquid starts coming out of the vent valve anyway.
The vent valve only needs to be open about 1/4 turn in many cases, and just a trickle of propane should be coming out. . .
