Turns out that it's not rocket science. There are no special trim removal tools, etc., just trap the plastic cover that extends from left of the steering wheel to right of the radio and pull. The individual clips will pop out and give limited access. The auxiliary 12vdc and cigarette lighter have to be disconnected to get any use access to the radio. This radio is mounted using 4 9/32 bolts.
Anyway, I was sure it was the 10 amp fuse on the back of the radio and it WAS NOT. Fuse is good. I rechecked and tightened all of the connections while I was there and put the radio back in the dash AND IT'S WORKING. Apparently one of the connections, probably the main plug, was loose.
Both radio and rear camera are working now.

I found a manual and there is a 10A fuse on the back of the radio. I'm guessing that's the problem.
Right now my problem is how to get the radio out of the dash. There's no access under the dash in a Ford E-450 and it's not apparent, at least to me, how to remove the radio. I'm sure that there's a bezel that must come off but I can't figure out how to remove it.
Anybody know how to remove a Pioneer AVH-P1400DVD from a E-450?
