The OP has a H21 receiver. It only has one cable coming to it. He claims he has setup this configuration many times before without having to change the options (SWM v. Multiswitch, etc.). If we take him at his word, we can ignore those devices as being the problem.
However the OPs' statement:

"Close enough" is OK for sex and hand grenades but not satellite TV. I don't know what he used for a meter (if any) but for sure it isn't a Bulldog or equivalent that verifies the satellite.
I believe he is pointed at the wrong satellite(s). There are several non-DirecTV satellites near 101. He needs to find the DirecTV 101 satellite, peak the signal, and if the skew is correct (or close), the receiver should see all the satellites.
