I have a Mediasonic HW-150PVR. It plays most flavors of video. It has most of the common video and audio outputs:
Video:
RF (Ch 3/4) both video and audio
Composite (RCA yellow) video only
Component (RCA red-green-blue) video only
HDMI both video and audio
Audio:
RF (Ch 3/4)
Stereo - RCA red(left) white(right)
Coax S/PDIF digital audio
HDMI
To get surround sound you would connect the HDMI or S/PDIF coax output to an audio system capable of processing these signals. The stereo RCA (red/white) outputs can not be used for surround sound.
It has one ATSC TV tuner and by adding a USB drive it can record OTA TV broadcasts. The recording is the same quality that is sent by the TV station. The highest quality currently being broadcast is 1080i Mpeg-2. CBS, NBC, and PBS typically broadcast 1080i. Fox and ABC broadcast 720p.
The Mediasonic box sells for about $33.
