Nordstrom, Ritz Carlton Hotels (All over the world) McDonalds (for consistency and general staff training on a world wide basis)Costco, EVA Airlines (Taiwan) Singapore Airlines, Thai Airlines, (all excellent on many many flights)
Many many companies know how to do staff training and followup to achieve a consistently good or excellent result. Good customer service is NOT about what mistakes you make it is about HOW you deal with those mistakes and how the customer is treated in the process.
I would add Southwest Air here is the US...their service is so good I fly them almost exclusively on domestic flights.
