I remember working with investigators after hundreds thousand dollars grain cargo was destroyed in ship cargo hold. First two ships of this type were build in Canada and were OK. Then Canada decided to build it in Shanghai.
Right off the wharf, maiden voyage had to be delayed for numerous reasons ranging from main engine not running (yes, I'm serious) to huge metal pulley in the cargo hold crumbling to pieces (bad metal, solly!), to wipers on the captain's bridge not working in heavy weather (was supposed to be Siemens unit, was labeled Siemens but somehow was filled with generic Chinese guts).
Later, in the middle of the passage - after they've loaded grain in another port - the fire broke out. There were powerful halogen lamps in the hold, near top. Big switch for those lamps was mislabeled and installed upside down, so it was left On while on the passage. Full hold of grain.
