Ford and GM use the CP4, which as you stated are prone to major failures resulting in big $$. FCA uses the CP4 in the EcoDiesel, Nissan uses it in their new Cummins Titan and VW uses it in their cars as well. In the US I think the only engine that still uses the more reliable CP3 is the I6 Cummins.
There was a guy who just in the last few weeks who had his 4 or 5 year old Duramax CP4 blow up on him, but GM stepped up and covered most of it. You can probably find that thread with a search.
