One question, you said your last truck was a 2003 with a 5 speed. Did one gear fall off? Pretty sure 03's had 6 speed sticks.
In 03, they had standard output and high output for the Cummins (similar to what they did with the 2nd gen 01.5 and 02). The standard output (250hp version) like I had came with the NV4500 5 speed and the high output had 305hp/555 lb ft and came with the NV5600 6 speed.

I knew they had two HP versions. Thought the 5 speed died with the 2nd gen trucks.
Minor detail, but there were actually 3 HP versions of the Cummins in 2003 if you count the California emissions one, and there was still a 5 speed (mine was one) if you got manual transmission and standard output engine. They also had two different autos that year; 47RE (definately early standard output autos, last part of the year may have changed to 48RE for all) and 48RE (all High output autos had the 48RE).
