As far as the OP's question on the valve stem, you do not need the stem facing IN. It was installed that way so the people who mounted those tires only had to mount the valve stem facing one way and the tire/wheel combo could be used on an inside dual or an outer. My Tiffin has the valve stems on the outer dual facing out so they are readily accessible. If you mount metal stems and need that tire mounted as the inner of a dual set, you can loosen the valve and turn it around without dismounting the tire from the rim.
As far as always replacing duals as a matched pair, this is also not true. I worked as a mechanic on big trucks for a while and we used to match dual tires by using a piece of rope to get mounted and inflated tires matched as closely as possible by circumference. We never had any issues with this procedure. When replacing a tire of a dual pair, be sure you get a brand that matches the circumference of it's mate if you cannot find an exact brand/series match . All tire guys know this but may sometime "forget" if you don't "remind" them
