I have been using NoMoRobo on both my landline and cell phone for quite some time (cell phone coverage just started last year). You need to id your carrier. The landline is free but there is a small cost for the cell phone per month. It is worth it! It is very rare that a call will get through that I don't want.
I always let my phones ring more than once. If it only rings once then it was a spam call. I have had not problems at all with the doctors, mail order pharmacy, etc. getting through.
The cell phone cost is $1.99/month or $19.99/year.
