As I said above, the illustrated adapter wiring is for a 50 to 30 amp adapter - it plugs into the 50 amp pedestal socket and only needs one leg tied to the 30 amp line. This adapter is used by RVs that use a 30 amp connection but may want to be able to connect to the 50 amp pedestal for a cleaner connection or because that is all that is available.
A 30 to 50 amp adapter plugs into the pedestal 30 amp socket and supplies that 30 amp hot to BOTH 50 amp hots. I have never heard of one that didn't work this way.
