Now thinking how that bracket on OP could break - I assume the TL ties have some play in the hole and when you drive without the camper - they move up and down. Over the time the bracket gets metal fatigue.

Different retainer for holding than I remember on my previous mounts
but unless the insert is really tight going into tube I could see some flex at the 'clips' bend. It does look like its been cracked, possibly even broke for a while.
