Others have told me they had a leak problem only if they turned the hose on while the black tank cap was locked on.

I think I will. I tried our flush connection once and it leaked so I just stopped using it. I'm not too fanatical about tank cleaning anyway. I just flush the toilet with a reasonable amount of water when it's used and I haven't had a problem in over 25 years (10 of them full time) of owning an RV.
My father in law is one of those who flushed his black tank with the valve closed, got distracted and about a thousand gallons later had a real mess inside.

Will it leak fresh water from the hose or dirty water from the tank?
Looks to me like it will leak a bit of dirty water from those holes marked "air inlet" in the drawing. Maybe even more than a little if it's below the level of the holding tank and gets a siphon action started via those "air inlets".

I think I see what you mean. I wonder why the manufacturers wouldn't just do that in the first place? Seems it would be cheaper than the valve they install under the cabinet.

straight thru inside the RV replacing the vent/check valve and put a vaccum break check valve on the outside. It screws onto the hose and then into the opening of the trailer. Does the exact same thing.
I don't think it does the same thing. The original was in place to prevent waste tank contents from siphoning from the tank and out thru the garden hose.
The one you installed (assuming it's a hose bib vacuum breaker like you would use on your house) is designed to keep the contents of the garden hose from siphoning back into the waste tank (house). It will allow waste tank contents to siphon out thru the garden hose and into the public water system.

If the water (fresh) is connected to the bottom in the illustration, then when the water pressure is removed and the plunger drops down, isn't the black tank now venting underneath the bathroom cabinet (or wherever it's mounted)? :E
