I appears someone removed the ambulance or U-haul body from a Ford cutaway chassis, fabbed up a cab extension with sheet meteal (the body lines are missing) and used a Dodge bed to finish the project off.

Cheap way to make a tow vehicle, ambulance chassis are cheap and readily availble. EMS units sell them after age, not mileage.

To the OP, did you ask the owner what was under the hood???
Maybe he Destroked the Ford van and in the process came up with a unique 5 th wheel hauler?
