Paying private for 3 months won't buy you any exemption from provincial rules. Provincial plan expires after 7 months total in calendar year. Then you return and stay 3 months on the private, re-apply for provincial plan, and leave the country again - and the plan will expire immediately, or will not be recognized if you try and get treatment within or outside Canada. Because you will not have stayed 7 months in that calendar year. I know of CRA circular on what to consider "close ties to Canada", but didn't know that we have to file such a form, and how often. My understanding is if you remain tax resident for CRA, and CRA doesn't allege that your status has changed, you don't need to file anything but regular tax return.
